What are tissue stimulators?

Tissue stimulators are a group of injectable products that do not fill wrinkles but encourage the skin to repair itself. The production of collagen, elastin and hyaluronic acid begins to decline at around the age of 25 — the tissue becomes thinner, loses support and starts to sag. Tissue stimulators are designed to address this process.

A filler works like a patch on a road: the gel enters the fold and lifts it immediately, so the effect is visible as soon as you leave the clinic and usually lasts for around a year. A stimulator does not produce an immediate result. It triggers controlled microinflammation, fibroblasts receive a repair signal and begin building new fibres over the following weeks. Your facial features remain your own — what changes is the quality and thickness of the skin. Biorevitalisation works in a similar direction, focusing mainly on hydration and the condition of the epidermis, while a tissue stimulator works deeper and helps rebuild the skin’s structure.

How do tissue stimulators work?

The product is injected into or beneath the dermis, where it remains as a foreign but fully biodegradable material. The body surrounds it with repair cells, fibroblasts increase their activity and produce type I and III collagen as well as elastin. The product itself gradually disappears, while the structures created by the skin remain.

  • The first changes in skin density appear 6–8 weeks after treatment
  • The full result develops after a course of treatments, usually in the third or fourth month
  • The effect lasts 12–18 months, and with poly-L-lactic acid it can last up to 2–2.5 years
  • Collagen produced by the skin remains in the tissue much longer than the product itself

One thing distinguishes tissue stimulators from hyaluronic acid even more than the speed of action: a stimulator cannot be dissolved. Hyaluronidase can remove a filler within several dozen minutes, but there is no equivalent option for tissue stimulators. That is why choosing the right product for the specific tissue and having it administered by an experienced practitioner are so important.

What does a tissue stimulator treatment involve?

The doctor injects the product with a needle using a technique similar to mesotherapy, or less commonly with a cannula when treating larger areas. Some products are administered through several dozen small injections across the entire treatment area, while others are injected into only 5–6 anatomical points on each side of the face — in the areas of the supporting ligaments. Fewer injections in precisely defined points mean a lower risk of hitting a blood vessel or nerve.

Preparation is required before the treatment. About two weeks beforehand, blood-thinning medications such as aspirin should be discontinued after consultation with your attending physician. Avoid sun exposure and tanning beds for one week, and avoid saunas and swimming pools during the final 48 hours.

At PIED-DE-POULE, the treatment takes:

  • 30 minutes — one area, for example the hands, eye area or a single scar
  • 60 minutes — the entire face
  • 90 minutes — the face with the neck and décolleté or a larger body area: abdomen, upper arms, inner thighs or the area above the knees

Each appointment includes a consultation, numbing cream, administration of the product and cooling of the skin after the procedure.

A standard course consists of 2–4 treatments approximately four weeks apart. Poly-L-lactic acid is administered less frequently — a repeat treatment is usually planned after 4–6 months. The first difference may be noticeable after just one appointment, but a lasting result usually requires a full course.

Tissue stimulators for the face

The face responds the fastest because this is where collagen loss becomes visible first: a sagging facial contour, less defined cheekbones, folds and thin under-eye skin. In addition to the face, the products can be used on the neck, décolleté, hands, upper arms, abdomen, inner thighs and the area above the knees.

The eye area has its own rules. The skin there is extremely thin and the blood vessels lie close to the surface, so only lightweight hydrating and collagen-stimulating products are used. Calcium hydroxyapatite and poly-L-lactic acid are not injected into this area because the risk of visible lumps is too high.

Types of tissue stimulators

The products differ in the strength of the stimulus they provide to fibroblasts. The stronger the stimulator, the longer the effect — and the more aftercare it may require from the patient.

  • Polynucleotides and Profhilo — the gentlest group; they activate fibroblast receptors, bind water and provide antioxidant effects; suitable for skin around the age of thirty without significant tissue volume loss
  • ZQ-II — a collagen-based product that stimulates the formation of new fibres locally over a small area; suitable for the eye area, scars and thin skin after the age of forty
  • Radiesse — calcium hydroxyapatite, a substance naturally present in the body; provides a slight lifting effect immediately after injection and lasts up to 1.5 years, making it suitable for people between 30 and 55
  • Juvelook — poly-L-lactic acid combined with hyaluronic acid; works on skin texture, acne scars and enlarged pores
  • Sculptra as well as AestheFill 1 / Powerfill 1 and AestheFill 2 / Powerfill 2 — the strongest collagen stimulators; the first option is intended for a single treatment area, while the second is designed for the entire face and neck, with results lasting up to 2–2.5 years

Which tissue stimulators are best?

There is no single product that is best for everyone. The right product is the one that matches the changes taking place in your skin.

  • Dull, dehydrated skin, first expression lines and no loss of volume — polynucleotides or Profhilo
  • Facial contours beginning to lose definition and flatter cheeks between the ages of 30 and 55 — Radiesse
  • Laxity, thin skin and noticeable loss of density after the age of forty — Sculptra or AestheFill / Powerfill
  • Acne scars, uneven texture and enlarged pores — Juvelook
  • Delicate eye area and localised scars — ZQ-II

Poly-L-lactic acid requires discipline: for two weeks after the treatment, you need to massage your face twice a day for five minutes. Failing to do so increases the risk of lumps developing at the injection sites. If you know you will not be able to maintain this routine, choose a gentler product.

Aftercare recommendations

The first 24 hours can determine how quickly the injection marks disappear.

  • Do not massage or rub the treated area during the first few hours unless your doctor advises otherwise in the case of poly-L-lactic acid
  • Avoid makeup for 24 hours
  • Change your pillowcase and face towel frequently and limit contact with pets while the injection points are still open
  • Apply SPF 50 for as long as needle marks remain visible — this helps protect against pigmentation
  • Avoid saunas, swimming pools and tanning beds for several days
  • After Sculptra and AestheFill / Powerfill treatments, massage your face twice a day for five minutes for two weeks, moving your hands along the muscle lines rather than across them — the same direction of movement is used during our facial massage

Redness and small bruises lasting for several days are a normal tissue response to the needle, not a complication.

Consultation and product selection

The doctor examines the skin, assesses its thickness, degree of laxity and areas where structural support has been lost. They ask about chronic conditions, medications, previous treatments and the date by which you would like to see the result. Based on this information, the doctor selects the product, determines the number of treatments and sets the intervals between them.

Skin preparation

Makeup removal, cleansing and disinfection of the entire treatment area. A numbing cream is applied to the skin and left under occlusion for approximately 20 minutes.

Marking the injection points

The doctor marks the injection sites while the face is both tense and relaxed because the tissue sits differently in each position. For products administered at individual anatomical points, the supporting ligaments are marked — usually five or six on each side.

Product administration

The product is injected with a needle or cannula, layer by layer and in small amounts. The injection stage itself lasts from around a dozen minutes to approximately thirty minutes, depending on the size of the treatment area.

Cooling and aftercare instructions

Cool compresses are applied to the skin to help reduce swelling and redness. You leave with a list of recommendations for the next 24 hours and a scheduled date for your next appointment.

Follow-up and the next treatment in the course

After approximately four weeks, the doctor assesses how the skin responded to the first dose and decides how much product should be administered during the next session. A course usually consists of two to four treatments, while with poly-L-lactic acid a repeat treatment is planned after 4–6 months.
